A Consent for Use of Name/Likeness is a form that should be used anytime you want to use a client’s or participant’s photograph, video, written testimonial, appearance and or likeness for any commercial purpose (e.g. on your website, on social media, on promotional materials, etc.)
So for instance, let’s say, you want to include a written testimonial with your client’s photograph on your sales page. Even if they’ve already provided the testimonial and image - through email or at a live event - you need to also have them sign this form for a couple of reasons:
- This protects you from being sued if that client were to come back and say you didn’t have permission to use their testimonial or image on Facebook.
- They know up front that they’re not being compensated for providing their testimonial.
